Day Habilitation – Supported Community Connections

We provide a wide variety of opportunities for adults with intellectual and developmental disabilities to become active members of the community. Our goal is to help the person we support to build meaningful relationships with others, and to pursue their passions. These activities will be conducted in a variety of learning settings in which the person in our services will interact with community members, not just their service providers and caregivers.

 

Homemaker

Eleos Services provides services to train and instruct the individual in basic homemaker tasks including cleaning, laundry or household care to increase independence of the individual. These services may include hands on assistance when the primary caretaker is unable to manage the home and care for the individual or there is a need for extraordinary cleaning.

 

Mentorship

At Eleos Services, we work with adults to build skills and confidence to include assistance in interviewing potential providers, understanding complicated health and safety issues, and assistance with participation on private and public boards, advisory groups and commissions. This service may also include training in child and infant care for parent(s) who themselves have an intellectual or developmental disability.

 

Non-Medical Transportation

Eleos Services will also provide transportation to adults, to safely get them to and from their home to jobs, community activities, etc.

 

Personal Care

Eleos Services will provide a range of assistance to accomplish tasks that the person would normally do for themselves: exercise routine, menu planning, dressing, grooming, money management, grocery shopping.

 

Respite

We provide short term, quality care for adults with intellectual or developmental disabilities, to give respite to their families or caretakers, and to enhance their quality of life. Respite may be provided in the community, or in the home of the respite care provider.
